for the a perfect perk progam

  1. In these terms & conditions a Perfect Perk means A Perfect Blend ABN 69 682 113 393 and includes, where appropriate, A Perfect Blends’ employees, agents, contractors & associated companies.
  2. Card means the a Perfect Perk Card, issued in accordance with these terms & conditions.
  3. Cardholder means a person who has applied for, and been issued with,
    a Perfect Perk Card
  4. A Perfect Perk means the member rewards programme run by A Perfect Blend.
  5. By applying for & accepting the Card, the applicant agrees to be bound by these terms & conditions and any variations to them; agrees to participate in A Perfect Perk and agrees to receive information from A Perfect Blend.
  6. To apply for the Card, the applicant must:
    • have previously signed up in store for the a Perfect Perk Friend Card, which will register your details; OR
    • have previously provided their details for an appointment, then visit the A Perfect Blend store from Monday 1st February 2010 to pick up your Card.
  7. Only individuals can apply for a Card. A Cardholder may hold 1 Card only.
  8. A Cardholders Card is for use by the Cardholder only.
  9. The Card is not a credit card, charge card or a guarantee card and is not transferable.
  10. The Card and any vouchers associated with the A Perfect Perk program remain the property of A Perfect Blend at all times.
  11. A Perfect Blend reserves the right to change the Card's terms & conditions without notice to Cardholders. Please check the terms & conditions viewable at aperfectblend.com.au regularly for any changes. Continued use of the Card will be deemed acceptance of those changes.
  12. To use the Card, Cardholders must present their Card to staff every time you make a purchase at A Perfect Blend.
  13. 10% off product applies to full rrp only. Excludes services and may not be used in conjunction with any other offer.
  14. However the 10% off product offer may be used in conjuction with
    A Perfect Blend's Gift Vouchers.
  15. A Perfect Perk prize vouchers are redeemable by simply presenting your Card instore at the time of a purchase by the Cardholder.
  16. A Perfect Perk prize vouchers must be used by the notified expiry (12 months after issue). Vouchers are for use by the Cardholder only and not transferable for cash.
  17. We recommend that Cardholders add the email perfectperk@aperfectblend.com.au to their email address book, to help prevent emails from A Perfect Blend being filtered out as spam or junk. All notifications from A Perfect Blend are sent by email. We take no responsibility for lost or misdirected emails.
  18. A Perfect Blend holds and used a Cardholder’s personal information for the purpose of managing the A Perfect Perk program. A Perfect Blend is bound by the Information Privacy Principles in the Information Privacy Act 2001 (Vic). All personal details of the entrants shall be stored and administered by A Perfect Blend. A request to access/update/correct/cancel any information can be presented in store or by calling 07 5438 0688 during business hours.
  19. A Perfect Blend is not responsible for stolen or lost Cards, including their fraudulent use or voucher redemption. The Cardholder must report a lost or stolen card to A Perfect Blend immediately on 07 5438 0688. The Cardholders card will be cancelled and a new Card issued.
  20. A Perfect Blend shall not be liable in any way if advertised benefits or promotions are unavailable as a result of technical malfunction, operator fault, short supply from supplier(s) or errors from computer hardware or software.
  21. To the greatest extent permitted by law, A Perfect Blend, gives no warranty and shall have no liability in respect of goods or services provided to Cardholders.
  22. A Perfect Blend makes no warranty or representation as to the quality, suitability or merchantability of any goods or services offered as prizes as part of the A Perfect Perk.
  23. A Perfect Blend may, at it’s absolute discretion & without providing reasons, accept or refuse an application for a Card.
  24. Cardholder rights are valid until terminated or cancelled by A Perfect Blend (in its absolute discretion) or until the Cardholder nominated A Perfect Blend that he or she wishes to terminate their card. This can be done in store or by calling 07 5438 0688 or by mailing to A Perfect Blend, Shop 19, 50 Bulcock Street, Caloundra, QLD 4551.
  25. For the avoidance of doubt, these terms & conditions supersede and replace any and all previous terms & conditions related to the A Perfect Perk program.
